Issued Nontidal Wetlands and Waterways Permit. A copy of the issued Permit, an MDE-cover letter, the Notice of Decision, Summary of the Basis for the Decision, Impact Plates, and Impact Tables are included within the below files.
Permit Conditions. The below-listed Special and General Conditions of the issued Permit reference various plans and materials. Those referenced plans and materials can be found at the links listed below each corresponding Permit condition. Hard copies of these plans and materials are also available for inspection at the following locations on or after August 11, 2015.

Special Condition E requires compliance with a Horizontal Directional Drilling Contingency Plan. A copy of that plan is provided in the below file.
Special Condition F requires compliance with a Dry Open-Cut, Dam-and-Pump Crossing Method Description. A copy of that description is provided in the below file.
Special Condition I requires implementation of MDE’s Best Management Practices for Working in Nontidal Wetlands, Wetland Buffers, Waterways, and 100-Year Floodplains. A document describing those practices is provided in the below file.
Special Condition J requires implementation of certain Enhanced Best Management Practices. A document describing those practices is provided in the below file.
Special Condition K requires implementation of certain Advanced Best Management Practices. A document describing those practices is provided in the below file.
Special Condition L requires compliance with Forest Conservation (FC) Plans and Forest Buffer Protection (FBP) Plans. Copies of those plans are provided in the below-listed files. To identify the files and pages applicable to each phase, please review the explanatory "FC and FBP Plan Files Cover Sheet" file at the top of the file list.

Special Condition O requires use of MDE spreadsheet MDE BIO3. A copy of that spreadsheet is provided in the below file.
Special Condition P requires compliance with a Hydrostatic Testing Discharge Permit. A copy of that permit is provided in the below file.
Special Condition Q requires compliance with a Spill Response Plan. A copy of that plan is provided in the below file.
General Condition 20 requires compliance with a Phase II Mitigation Plan as approved by MDE. Copies of that plan and approval letter are provided in the below files.
